The ZFA shortage is partly a salary problem. Many practices are still paying rates set years ago without adjusting for inflation, the tightened labor market, or the new collective agreement benchmarks. Candidates applying today frequently hold competing offers — and the practices that move fastest with competitive packages win the best applicants.

Current ZFA salary benchmarks in Germany (2026)

Entry-level ZFA (0–2 years experience): €2,100–€2,500 gross per month in most regions, with Munich and Hamburg premium of 8–12%. Experienced ZFA (3–7 years): €2,500–€3,100 gross. Senior ZFA with additional qualifications (DH, ZMV, ZMF): €2,900–€3,600+ gross.

Regional differences

Practices in metropolitan areas need to benchmark against local cost of living, not national averages. A ZFA in Munich managing a 30-patient-per-day schedule in a high-performing private practice can reasonably expect €3,000+ gross, while a smaller rural practice may find well-qualified applicants at €2,300–€2,500.

Beyond salary: what ZFAs are actually looking for

In candidate surveys, ZFAs consistently rank the following as important as salary: regular working hours without last-minute shifts, a functional and modern workspace, genuine continuing education support, and a team culture without hierarchy-driven tension. A practice that pays market rate but invests in these factors will outperform one paying above market in a difficult team environment.
